Nurse Practitioner, HouseCalls in Lake and Orange counties, FL

About the position Optum is seeking a Nurse Practitioner to join the HouseCalls team in Lake and Orange counties, FL. This role involves providing annual clinical assessments to patients in their homes, focusing on preventive care to identify and reduce health risks. The Nurse Practitioner will coordinate follow-up care to enhance patient health and well-being, working within an integrated medical team to deliver high-quality, compassionate care to vulnerable populations. The position offers flexible scheduling with no on-call, weekends, or holidays required, promoting a healthy work-life balance. Responsibilities • Conduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam. , • Perform evidence-based practice screenings including point of care testing as appropriate. , • Identify diagnoses for care management and active medical management. , • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings. , • Communicate assessment findings to inform the member's PCP of potential gaps in care. , • Identify urgent and emergent situations and intervene appropriately. , • Educate members on topics such as disease processes, medication, and compliance. Requirements • Active NP license or ability to obtain by start date, unencumbered and free of disciplinary actions. , • National certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or ability to obtain by start date. , • Ability to complete physical requirements of the job including lifting a 30-pound bag, navigating stairs, and performing physical assessments. , • Reliable transportation for home visit assessments. Nice-to-haves • 1+ years of clinical experience in their highest level of education, clinical setting. , • Experience in gerontology, cardiology, internal medicine, or endocrinology. , • Home health care or home visit experience. , • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
