Job Overview As an MDS Coordinator, you will be instrumental in leading the Minimum Data Set (MDS) process, ensuring that assessments for all residents are executed accurately and punctually. You will play a vital role in guiding the interdisciplinary team, promoting optimal resident care while identifying trends through data analysis to implement necessary quality improvement initiatives. Key Responsibilities Lead the MDS process, ensuring precise and timely assessments for every resident. Supervise the interdisciplinary team, offering guidance and support to guarantee the highest standards of resident care. Analyze resident data to identify patterns and spearhead quality enhancement initiatives. Ensure adherence to all federal and state regulations. Cultivate a collaborative team environment to support the care team's efforts. Required Skills Active RN license in good standing. RAC-CT Certification. Demonstrated experience in completing and overseeing MDS assessments. Exceptional leadership, communication, and analytical skills. A genuine passion for delivering high-quality care to seniors. Qualifications An active Registered Nurse (RN) license. RAC-CT Certification. Proven experience in MDS coordination within long-term care settings.
