UNIT PROFILE The Nurse-Led Outreach Team (NLOT) is a nursing team that is made up of Registered Nurses (RNs), with expertise in geriatrics and seniors’ mental health who provide timely access to evidence based, quality care for older adults living with complex health conditions in the Long-Term Care (LTC) setting. This team provides mobile urgent assessment to residents in their LTC homes for acute and episodic conditions and responds to changes in condition and functional decline to support palliative and end-of-life care in a consult model. NLOT programs reduce unplanned transfers to emergency departments (ED) and hospital admissions, while providing care in the home aligned with resident goals, values and wishes.

POSITION SUMMARY The Registered Nurse (RN), as a member of the Nurse-Led Outreach Team (NLOT), will expand capacity in the LTC homes among staff and clinicians by provide education and training on various relevant topics. This individual will also provide time-sensitive assessments in the LTCs to reduce preventable transfers to the ED, hospital admissions, and reduce hospital LOS. The NLOT RN will identify and facilitate direct access to the right ambulatory services based on assessed needs by working collaboratively with an inter-professional team, family members, support network, community partners, and a variety of informal and formal service providers. 

The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty standards. The RN provides patient/client-centered and evidence informed care to promote health and assess the need for the provision of care and the treatment of health conditions. The RN collaborates with the inter-professional team and assumes a leadership role in the delivering a full range of care and treatment to patient/clients including those with complex needs, high acuity and less predictable outcomes.

QUALIFICATIONS • Current registration with the College of Nurses of Ontario required • Current BCLS/HCP certification • RNAO, CNA or other professional association membership preferred • Minimum 3 years of recent related clinical experience working with older adults • Training in Comprehensive Geriatric Assessment is preferred • Wound care and assessment i.e. strong knowledge and skills assessing wounds and making recommendations to LTC staff and liaising with treating physician preferred  • IV PICC line management i.e. ability to manage all aspects of IV PICC care and train LTC staff on same including problem solving and supporting understanding of management preferred • Ability to complete STAT bloodwork preferred • Strong clinical assessment skills required. Previous experience in Emergency Department, Stepdown Unit, or Acute Care is an asset. • Previous experience in Emergency Department, Stepdown Unit, or Acute Care is an asset. • Demonstrated ability to utilize principles of knowledge transfer to facilitate practice change & implementation of best practices • Gentle Persuasive Approach training preferred. • CPI – Crisis Prevention Training (or successful completion as part of clinical orientation) required. • Comprehensive knowledge of community resources and health care delivery systems in acute and community settings. • Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families. • Demonstrated ability to respond to clients and situations with flexibility and adaptability. • Demonstrated knowledge and practice of safe and secure environment for clients/families/teams by listening, discussing, exploring, clarifying and acting on their concerns, needs and priorities required. • Demonstrated expertise and competence in areas of client need, including: chronic disease management, mental health and addictions, complex and dysfunctional family systems, older adults who are frail and/or palliative, medication and symptom management for specific diseases. • Excellent interpersonal, communication, organizational and decision-making skills required. • Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution and negotiation . • Demonstrated ability to work independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers. • Excellent time management and stress management skills required. • Demonstrated knowledge of and experience in the application of adult education principles preferred. • Demonstrated knowledge and understanding of P.I.E.C.E.S model of assessment and ability to apply preferred. • Knowledge of the Occupational Health and Safety Act and Regulations, professional standards and guidelines, SJHH corporate and departmental Policies and Procedures, including successful completion of WHMIS training required. • Knowledge of documentation standards, privacy legislation, and health record management processes required. • Computer literacy and basic proficiency is required. • A valid driver’s license, adequate insurance (minimum of $5 million personal liability) and use of a reliable vehicle required.

RESPONSIBLITIES • Communicates effectively with all staff, including the inter-professional team, family members, support network, community partners, and a variety of informal and formal service providers. • Provides direct patient care for patients. • Follows infection control guidelines during patient care. • Works with management and multidisciplinary team to implement best practice and organizational mandates. • Provides mobile urgent assessment to residents in their LTC homes for acute and episodic conditions  • Responds to changes in condition and functional decline to support palliative and end-of-life care in a consult model • Assists with training new staff as needed. • Assists with student learners as needed. • Acts as a role model for the profession and hospital.  • Keeps current with legislation and guidelines for Registered Nurses through the College of Nurses • Keeps current with CPR certification • Adheres to infection control & safety guidelines for staff information & compliance • Adheres to of hospital and departmental policies
