As a Patient Access & Referral Specialist, you will:

• Conduct mobile assessments and review for clinical disposition.
• Provide crisis intervention support.
• Triage patients as needed.
• Complete assessments and provide referrals as appropriate.
• Collaborate closely with medical staff, treatment teams, and managed care organizations.

Requirements:

• Master's Degree in Social Work or related field; or Registered Nurse license in the State of Indiana.
• Proficient in computer applications.
• Excellent listening and communication skills.
• Strong organizational and time management abilities.
• Ability to work independently and as part of a team.
