Care Partner - Remote

Key Responsibilities:

• Provide comprehensive social and medical case management support to members across the U.S.
• Manage and prioritize a caseload of member cases, ensuring timely and effective interventions.
• Conduct thorough assessments to determine member eligibility for case management services.
• Connect members with appropriate resources, including specialty programs, educational materials, and community support services.
• Conduct research and provide guidance on member issues, including referrals to legal, financial, and community support services.
• Collaborate with colleagues and clinical staff to achieve optimal member outcomes.
• Maintain accurate and complete documentation of all member interactions and case details.
• Participate in quality improvement initiatives to enhance member care.

Qualifications:

• Proven experience in social and medical care.
• Strong customer service and communication skills.
• Proficiency in using computers and navigating various systems.
• Bachelor's degree or equivalent experience.

Preferred Qualifications: Prior experience in managed care, psychology, or social work; Bilingual abilities.
