Per Diem Field Licensed Vocational Nurse /LVN
• THIS IS NOT REMOTE, THIS IS ON THE ROAD (FIELD WORK)* The Doctor’s Choice Home Health is currently seeking experienced and dedicated Licensed Vocational Nurses to join our Home Health team within the Inland Empire Area. This skilled nurse will be able to make visits in the patient’s place of residence, in order to provide skilled care services as ordered by referring physician. Per-Diem Employment Status MUST be able to see patients in Chino, Corona, Rancho Cucamonga, Upland and Montclair. Minimum Requirements include:
- Graduate of a National League for Nursing accredited school of nursing
- Must have minimum 1 year of experience in nursing as an LVN
- Able to see patients at least 3 days a week
- Experience in EMR software (preferred but not required).
- Provide skilled nursing services/interventions in accordance with physician orders.
- Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
- Collaborates with all disciplines to plan and evaluate team goals for each patient.
- Experience with Wound Care
Required Licenses and Certifications:
- LVN License in California
- Valid Driver’s License & Auto Insurance
- Current CPR certification
- Physical and TB from within the last 6 months
