As the clinical team leader for our home health, palliative care, and hospice teams, the Nurse Practitioner will be responsible for providing comprehensive patient care in a home environment. Responsibilities ? Carry out complete patient evaluations and coordinate with facility staff to enhance preventive care efforts and promptly identify any early signs of health deterioration. ? Address acute and persistent illnesses by making changes to medication usage. ? Organize and decipher diagnostic procedures. ? Develop, execute, and refine care regimens as needed. ? Be involved in quality assurance and enhancement endeavors. ? Provide clinical instruction and guidance to patients, families, and facility staff to improve their understanding of medical conditions and treatments. ? Abide by the state-mandated scope of practice rules applicable to nurse practitioners. ? Exhibits expertise in identifying the sources, susceptibilities, bodily responses, and signs of both immediate and enduring ailments. Qualifications ? Eligibility criteria include having either a Master's degree in Nursing or an active RN license. ? The Advanced Practice Nursing license grants permission to provide elevated levels of care and make independent clinical decisions. ? Nurse Practitioner credentialing by a recognized certifying body. ? It is advantageous to have the necessary credentials recognized by insurance firms. ? Fingerprint clearance. ? Individuals must possess a DEA license in order to have unimpeded authority to prescribe and dispense medications. ? This job necessitates holding a driver's license issued by the state and providing proof of insurance. ? Preference is given to individuals with a background of 1-2 years in nurse practitioner roles ? Candidates who have prior experience with adult and geriatric populations and a solid understanding of population health management will be prioritized ? Demonstrates strong communication, organizational, and customer care competencies ? It is beneficial to have knowledge about Skilled Nursing/Transition Rehabilitation Centers ? Effectively dealing with a significant influx of patients with expertise and assurance ? Mastery and implementation of Electronic Health Records in practice ? Capability to work with private information, notably data safeguarded by HIPAA guidelines ? Ability to be proactive and initiate processes with little to no supervision Benefits ?
