Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences) and our independent living community (Fountaingate Gardens). Our Certified Home Health Agency and Gurwin Licensed Home Care Services Agency have an office in Farmingdale, NY POSITION: part-time Licensed Practical Nurse DEPARTMENT: Nursing HOURS: 7:00am-11:00am, 8:00am-12:00pm or 9:00am-1:00pm (5 shifts per week including every other weekend) Requirements include: • Graduate of an accredited professional nursing program with a current license to practice in New York State. • Demonstrates interest in the special needs of geriatric patients and possesses an ability to integrate principles of geriatric nursing into nursing practice. • Ability to establish effective interpersonal relations with patients, staff and families. • Ability to communicate effectively in English, both orally and in writing. • Education in all areas of LTC/Subacute to ensure competency on all units in the facility. Responsibilities include, but are not limited to: • Participates in the nursing care of patients within the scope of the legal definition of the practice of practical nursing. The LPN works under the supervision and/or direction of a registered professional nurse. • Participates in the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients under the supervision of an RN. • Administers patient care as assigned by an RN within departmental guidelines. As the unit situation requires, an LPN may be assigned to assume team leader, primary nurse or charge responsibilities under the direction of an RN • Directs and supervises non-professional staff as assigned. • Observes, records and reports patient data as dictated by departmental policy. • Participates in unit nursing and inter-disciplinary team activities. • Administers medications and performs nursing procedures as dictated by departmental policy. • Attends nursing and other institutional in-service programs and participates in departmental committees as assigned. • Performs related and miscellaneous duties as directed.
