Job Summary To ensure effective patient and family-centered care delivery in the hospital setting, care managers provide clinically-based care management. They coordinate discharge planning and follow-up care, establish realistic goals, and promote quality patient outcomes through collaboration with medical staff and the interdisciplinary team. Job Description

Essential Functions: • To provide the highest standard of service, assistants should make certain to keep abreast of the most recent developments in reimbursement procedures, managed care contracts and patient status when collaborating in the plan of care. • Collaborates with internal and external stakeholders to ensure that provider-related activities are aligned with organizational goals and objectives. • The assistant is involved in maintaining compliance with Utilization Review and regulatory requirements by identifying trends. Collaborating with hospital staff and external organizations is needed to make necessary process changes. • Plays a crucial role in supporting the medical staff quality review process by identifying quality issues emanating from the medical staff quality indicators. • Creates a personalized Plan of Care (POC) that is tailored to the patient and family's requirements and updates it as necessary to reflect any changes in those requirements. • Utilizes accurate and comprehensive documentation practices to communicate effectively and efficiently with healthcare professionals, patients, and families in making informed health decisions. • Promotes efficient planning for patient and family needs, allowing for a coordinated and effortless transfer between healthcare providers. • The assistance, along with the medical team, evaluates the current treatment plan to identify barriers and establish goals. Collaborative efforts with support services departments ensure successful transitions during the patient's discharge. • Is a visible resource on the unit and coordinates interdisciplinary team huddles/communication, identifying and communicating the patient’s health care needs based on the best practice standards to ensure care and communication needs are met in relation to both internal and external providers/services, ancillary department services, core measures, compliance with internal policies/regulatory guidelines and True North metrics. • All other duties as assigned. Additional Requirements Required: Education - Completion of a nursing education program that is approved by a board of nursing Certification - Registered Nurse (RN) - South Dakota Board of Nursing Preferred: Education - Bachelors in Nursing Experience - 5+ years of Acute Experience; 5+ years of Registered Nurse Experience Certification - Commission for Case Management Certification (CCMC) - Accredited University or accredited training professionals Physical Requirements: Medium work - Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
