Care Coordinator/Remote Patient Monitoring Programs (RPM)

Our Remote Patient Monitoring (RPM), Remote Therapeutic Monitoring (RTM), and Chronic Care Management (CCM) programs are at the forefront of how we extend that care — meeting patients where they are, monitoring their health between visits, and keeping them connected to their care team every single day. We’re looking for a dedicated Care Coordinator to help us do that work well. As a Care Coordinator for our RPM/CCM programs, you’ll be the primary point of contact for patients managing chronic and complex conditions — people living with diabetes, hypertension, heart disease, and more. You’ll monitor their biometric data daily, check in with them regularly, educate them on their devices and care plans, and make sure that when something looks off, the right people know right away. This is a full-time, onsite role based at our Back of the Yards and Halsted locations. Here’s what your days will look like Monitor daily patient data — vitals, readings, and biometric trends — through RPM and CCM platforms. Serve as the primary point of contact for enrolled patients, providing consistent guidance, education, and encouragement. Conduct regular virtual check-ins, update care plans, and support patients in self-managing their conditions. Identify concerning trends or critical alerts in patient data and escalate promptly to the provider team. Coordinate appointments, tests, procedures, and referrals to keep care moving forward without gaps. Assist with insurance authorization and referral processes as needed. Connect patients with community resources, social services, and support programs — transportation, financial assistance, food services, and more. Advocate for patients’ preferences, rights, and needs throughout the care process. Maintain accurate, timely documentation of all care coordination activities in EMR. Generate reports on patient progress, program metrics, and outcomes for leadership and the clinical team. Facilitate communication and information exchange among providers, specialists, and community partners. Collaborate closely with physicians, nurses, and support staff to ensure continuity of care. Track and report on key performance indicators (KPIs) related to readmission rates, screening compliance, and care coordination effectiveness. Participate in quality assurance, peer reviews, and workflow improvement initiatives. Train new staff on RPM/CCM workflows and platform use. Monitor resource utilization and identify opportunities for cost-effective, high-quality care delivery. #### You’ll thrive in this role if you bring Active Licensed Practical Nurse (LPN) license in the State of Illinois with a minimum of 2 years of clinical experience; OR a Certified Medical Assistant (CMA) with 7+ years of experience in a high-volume clinical environment #### Current BLS certification (required) Previous experience in case management, telehealth, or remote care coordination (required) Familiarity with RPM/RTM/CCM platforms and digital health monitoring tools Familiarity with HIPAA, OSHA, and healthcare compliance basics Proficiency with EMR systems — eClinicalWorks (ECW) experience is a strong plus — and Microsoft Office Suite Deep knowledge of chronic disease management principles and healthcare delivery systems Exceptional communication skills — empathetic, clear, and active listening are non-negotiables here Strong organizational and time management skills with sharp attention to detail Ability to work independently, exercise sound clinical judgment, and manage multiple patient priorities simultaneously Demonstrated cultural competence and genuine ability to connect with patients from diverse backgrounds Excellent relationship-building skills — patients and care teams alike will count on you Bilingual in English and Spanish?
