Your outreach helps patients better manage chronic conditions, understand preventive care, and stay engaged in their health goals. Over 75% of your day is spent speaking directly with patients, offering education, compassion, and clarity. This is not a job where you wait for the phone to ring—you drive the connection. What You’ll Do
- Make up to 20 outbound patient calls daily
- Conduct assessments within your LPN scope to understand each patient’s unique needs
- Create and collaborate on personalized care plans
- Serve as the communication bridge between patients, families, and providers
- Educate patients on their conditions, treatment options, and preventive care
- Track progress and adjust care plans as needed
- Document interactions and work within multiple healthcare systems and tools
What You Bring Required
- Active LPN license
- 2+ years of hands‑on patient care experience
- A HIPAA‑compliant home office with secure high‑speed internet
- Experience in patient care coordination or case management
