Referral Nurse Navigator (RNN) Fully Remote | Contiguous United States | Monday–Friday, Day Shift Our client, a technology-enabled clinical services company focused on improving access to specialty care, is expanding its Referral Nurse Navigator (RNN) team in response to significant client growth and the launch of expanded service offerings. This is a fully remote, direct-hire opportunity with a mission-driven organization that supports primary care providers through eConsults, telehealth, in-person specialty care, care coordination, health coaching, and analytics. The RNN role is central to Our Client's care model. As the clinical liaison between primary care providers, specialists, and the broader care team, the RNN ensures that referrals are accurately packaged, routed, and resolved — driving better outcomes for patients and a more efficient experience for providers. This is an individual contributor role with a clear pathway to Supervisor-level leadership as the team scales toward 30-plus RNNs. Who You Are You are an experienced Registered Nurse who thrives in a focused, desk-based remote environment and takes pride in producing clean, accurate clinical work. You know "a little about a lot" rather than a lot about one narrow specialty, and you are confident reviewing charts across pediatric, adult, and geriatric populations. You are tech-savvy, comfortable navigating multiple EHRs and remote desktops, and quick to learn new platforms. You communicate clearly in writing, understand that your documentation becomes part of the legal patient record, and you are diplomatic when engaging with physicians. You are a self-starter who exhausts internal resources before escalating, owns mistakes as learning opportunities, and welcomes constructive feedback. You enjoy a collaborative, Slack-driven team culture, but you stay focused and accountable to daily throughput expectations. You treat remote work as a professional commitment — not a flexible side arrangement. The organization operates a PCP-centered, whole-person, longitudinal care model designed to support — not replace — primary care providers. The culture is collaborative, transparent, and accountability-driven. Team communication runs through Slack, with topic-based sub-channels and a morning check-in rhythm. RNNs are recognized as the number-one internal end users of the company's proprietary eConsult platform and are actively encouraged to share product feedback that shapes the tool's evolution. Retention on the RNN team has historically been exceptional, with most departures resulting from internal promotion or geographic moves rather than turnover. Your Role As a Referral Nurse Navigator, you will:

  • Review assigned referrals daily (approximately 60 per day) and submit the eligible subset (approximately 35 per day) as eConsults, packaged with full clinical context for specialist review.
  • Serve as the clinical liaison between primary care providers, specialists, and the internal team, ensuring that each eConsult contains the information specialists need to provide accurate recommendations.
  • Route referrals appropriately, follow up on saved or pending eConsults, monitor overdues, and notify PCPs of specialist responses.
  • Conduct thorough chart reviews to surface the clinical context required for each consult.
  • Track personal performance through visible dashboards covering throughput, time-per-consult, and peer benchmarks.
  • Identify workflow improvements and contribute product feedback to engineering as a primary end user of the platform.
  • Collaborate in real time with peers across Slack sub-channels in an "all hands on deck" team environment.

Schedule: Monday through Friday, 8:30 AM – 5:00 PM CST, with approximately 30 minutes of flex. No mandatory overtime, no on-call, and no weekend shifts. Limited holiday coverage is required when client clinics are open. Travel: None. This is a fully remote role with no direct patient care. How You Qualify Required:

  • Active, unrestricted Registered Nurse (RN) license in any U.S. state (Associate or Bachelor's degree acceptable).
  • Minimum of two years of RN experience.
  • Generalist clinical exposure across multiple conditions and patient populations (pediatric, adult, and geriatric).
  • Strong written communication and grammar.
  • Comfort working across multiple EHRs and remote desktop environments.
  • A dedicated, private, HIPAA-compliant home office that meets the following standards:
  • Screen is not visible to others in the household.
  • Voice conversations regarding patient cases cannot be overheard.
  • No work performed from public spaces (coffee shops, libraries, restaurants, or similar venues).
  • Consistent childcare arrangements if minors are at home during work hours (infants and pre-school-age children at home during work hours are not compatible with the role).
  • Secure, private high-speed Wi-Fi; backup hotspot recommended.
  • Screen locked when stepping away from the workstation.
  • Desk-bound, multi-monitor workstation setup (equipment provided).

Preferred:

  • EPIC or AthenaHealth experience, ideally at a super-user level. Additional EHR experience (Cerner, Meditech, Allscripts) is a plus.
  • Background in chart review, utilization management, care coordination, or referral coordination.
  • Prior remote work experience with demonstrated focus discipline.

Not required:

  • BLS certification or additional clinical certifications.
  • Compact licensure (no direct patient care is performed).
