About the Role

We’re looking for a compassionate and detail‑driven Registered Nurse (RN) to join our team as a Case Manager RN. In this role, you’ll help guide patients through their healthcare journeys, ensuring seamless transitions, advocating for their needs, and improving outcomes through thoughtful care planning and collaboration. What You’ll Do Coordinate and manage patient care plans across healthcare settings Assess patient needs using clinical data, history, and diagnostics Collaborate with providers and care teams to ensure effective communication Support safe, timely transitions from hospital to home Conduct utilization reviews to promote high-quality, cost‑effective care Track patient progress and manage disease and case management workflows Leverage digital tools and analytics to monitor performance and outcomes Connect patients with community resources to enhance care Working scheduled is 8:00 am to 4:30 pm in your time zone What We’re Looking For - Required Active & unencumbered RN license (required) in the state of residence. 3+ years of acute care clinical experience as an RN Strong problem-solving and decision-making skills Knowledge of medical terminology and clinical workflows Comfort with digital tools and healthcare systems Ability to communicate clearly and handle sensitive information with care Passion for patient advocacy and working with diverse populations Preferred A compact RN license is preferred Managed care experience preferred Home Care experience preferred Certified Case Manager (CCM) preferred Associates Degree in Nursing required, BSN preferred

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
