Responsibilities

• Evaluate and treat patients in a home setting.
• Communicate with families, physicians, and other health care professions.
• Maintain daily thorough documentation of services in the EMR.
• Have your own equipment including smart devices and blood pressure monitor.
• Treat hospice patients.

Qualifications

• Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy.
• Education and/or Experience: New graduate applicants welcome + Experienced Therapists.
• Actively Licensed as a FL Physical Therapist.
