As an HRA Nurse Practitioner at Cigna, you will have the opportunity to make a meaningful difference in the lives of our members by conducting health risk assessments and providing personalized care plans. To excel in this role, you must be a licensed and experienced Nurse Practitioner with a strong background in primary care and a passion for patient-centered care. Conduct health risk assessments for Cigna members in a professional and thorough manner. Develop personalized care plans for members based on the results of health risk assessments. Collaborate with other healthcare professionals to ensure the best possible care for members. Maintain accurate and thorough medical records of all assessments and care plans. Utilize evidence-based practices to provide high-quality and compassionate care to members. Ensure compliance with all applicable laws, regulations, and company policies in regards to patient care. Communicate effectively with members to educate them on their health risks and care plans. Participate in ongoing training and development to stay up-to-date on best practices and industry standards. Utilize technology and data to track and monitor the progress of members' health. Act as an advocate for members and promote a patient-centered approach to care. Collaborate with other healthcare professionals to identify and address any gaps in care. Maintain a high level of professionalism and confidentiality at all times. Provide exceptional customer service to all members and address any concerns or questions they may have. Continuously evaluate and improve upon processes and procedures to enhance the quality of care provided.
