Job Responsibilities:

As a licensed social worker in this remote case management role, you will:

  • Collaborate with registered nurses to assess patients' needs and develop personalized care plans
  • Research and identify available resources to address patients' financial, transportation, safety, and housing needs
  • Empower patients to make informed lifestyle and medical choices through education and advocacy
  • Work closely with other clinical staff to provide comprehensive support and resources to patients
  • Utilize telephonic and electronic communication to engage with patients and stakeholders
  • Maintain accurate and up-to-date records of patient interactions and progress

Requirements:

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

  • 2+ years of experience in medical social work or case management
  • Master's degree in social work or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in electronic medical records and other relevant software systems

* Competitive salary range: $46,597.20 - $101,228.40
