Job Summary

CVS Health is seeking a compassionate and detail-oriented Patient Care Coordinator to join our dynamic healthcare team. In this remote role, you will serve as the primary point of contact for patients, ensuring their healthcare experience is seamless, supportive, and efficient. Key Responsibilities

  • Serve as the first point of contact for patients via phone, email, and digital communication platforms.
  • Coordinate and schedule patient appointments, follow-ups, and necessary healthcare services.
  • Maintain accurate patient records, ensuring compliance with HIPAA and internal data protection policies.
  • Address patient inquiries and concerns promptly, providing clear information and guidance.
  • Collaborate with healthcare providers, pharmacies, and insurance teams to facilitate patient care.
  • Assist patients in navigating benefits, insurance coverage, and care options.
  • Document and report patient interactions, escalations, and follow-up actions.
  • Participate in team meetings, training sessions, and process improvement initiatives.

Required Skills and Qualifications

  • High school diploma or equivalent; Associate’s or Bachelor’s degree in Healthcare, Administration, or related field preferred.
  • Strong verbal and written communication skills.
  • Exceptional customer service skills with empathy and professionalism.
  • Proficiency in Microsoft Office Suite and familiarity with electronic health record (EHR) systems.
  • Ability to multitask and manage time effectively in a remote work environment.
  • Strong organizational skills and attention to detail.

Experience

  • Minimum of 1–2 years of experience in patient services, healthcare coordination, or customer service within a healthcare setting preferred.
  • Experience working remotely is an advantage but not required.

Working Hours

  • Full-time, remote position with flexible working hours.
  • May require occasional evening or weekend coverage depending on patient needs.

  • In-depth understanding of patient care processes and healthcare terminology.
  • Ability to handle sensitive patient information with confidentiality and discretion.
  • Problem-solving skills with the ability to provide practical solutions quickly.

  • Self-motivated, independent, and capable of thriving in a remote work environment.
