Job Summary

As a remote social worker, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a population spanning the entire lifecycle, utilizing telephonic or electronic exchange to connect with members and empower them to make positive lifestyle and medical choices.

Key Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs

  • Research available options and resources for members with financial, transportation, safety, and housing needs
  • Utilize telephonic or electronic exchange to connect with members and empower them to make positive lifestyle and medical choices
  • Work with a population spanning the entire lifecycle, including children, adults, and seniors
  • Develop and implement individualized plans to address members' needs and promote positive outcomes
  • Collaborate with other clinical staff to connect members with community resources for medical, financial, and behavioral health
  • Maintain accurate and up-to-date records of member interactions and progress

Essential Qualifications

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence

  • 2+ years of experience in medical social work or case management
  • Masters degree in social work or a related field

  • Knowledge of community resources and services, including medical, financial, and behavioral health
  • Strong communication and interpersonal skills, with the ability to build rapport with members and collaborate with clinical staff

  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions
