[Remote] In-Home Dialysis Care Partner (1:1 Client) – Fluent Spanish Required

Note: The job is a remote job and is open to candidates in USA. Herewith GmbH is seeking a compassionate and reliable Care Partner fluent in Spanish to support in-home dialysis for a patient transitioning from clinic to home care. The role involves providing safe and high-quality dialysis support, building trust with the patient, and ensuring a smooth care transition.

Responsibilities

  • Assist with in-home dialysis treatment setup (NxStage or similar systems)
  • Maintain a clean, organized, and safe treatment environment
  • Support the patient in staying on schedule with dialysis treatments
  • Provide companionship and emotional reassurance
  • Assist with light Activities of Daily Living (ADLs) as needed
  • Monitor patient safety and report concerns to the coordinating nurse
  • Maintain simple documentation and shift notes

Skills

  • Minimum 1 year of dialysis experience (home or clinic)
  • Fluent in Spanish (required)
  • Open to in-home dialysis care and completing provided training
  • Calm, patient, and dependable—especially during care transitions
  • Strong communication skills and ability to collaborate with a clinical team
  • Must pass a Herewith background check
  • Familiarity with home dialysis systems (NxStage a plus)
